We are located inside The Livery Building in Upper Montclair and provide care by appointment only.

## Finding the Office
1. Use the Exact Address in Your GPS
For the most accurate directions, enter the address exactly as listed above. If your GPS routes you behind the building or to a different street, continue until you reach Cooper Avenue.
2. Locate The Livery Building
The Livery Building is located across from Starbucks and CVS and next to Jason's Tire Shop. Pelvic PT Plus is located inside the building rather than in a street-facing storefront.
3. Enter Through the Side Entrance
Please use the side entrance to access the building. Once you enter the hallway, Pelvic PT Plus is located in Suite 5D.
4. Check In Upon Arrival
As you enter the hallway, you will see a Pelvic PT Plus "Check In" poster. Please scan the QR code using your smartphone to complete your arrival check-in.
5. Waiting Area
After checking in, please have a seat in our waiting area located directly across from Suite 5D in Suite 5BC. Your physical therapist will come to greet you when it is time for your appointment.
Parking Information
Street parking is available along Cooper Avenue and nearby streets. There is one accessible parking space located directly across from the side entrance.
Please do not park in reserved or tenant-designated spaces. Additional public parking is available in the municipal lot across Valley Road.
Before Your Appointment
* Please arrive 5–10 minutes before your first appointment.
* Complete all intake paperwork prior to your visit whenever possible.
* Wear comfortable clothing that allows for movement and physical assessment.
* If you have relevant imaging reports, physician referrals, or medical records, please bring them with you or upload them before your appointment.

Pelvic PT Plus provides private, one-on-one treatment sessions focused on pelvic health, pregnancy and postpartum recovery, pelvic pain, bowel and bladder dysfunction, sexual health concerns, pediatric pelvic health, and orthopedic conditions. Every treatment session is individualized to your needs and goals.
